How do I edit/review my incomplete charts?

This section is to briefly define the procedure for processing and filing discharge summaries and operative reports.


Physicians and clinicians are normally contacted via mail or fax or directly in the Patient Care Information System (PCIS) regarding charts that are incomplete or need to be filed according to the regional Standard Operating Procedure (SOP) schedule. 

Within the correspondence, a timeline will be set for your response. If you would like to contact us about the deficiency report procedures associated to your site, please use our contact information, listed below.

 

Charts are available for review in the Records Management (Health Records) department. Reports are available for viewing in PCIS; all report edits are handled by Transcription Services via the Fluency (M-Modal) system. 

 

Any questions regarding incomplete charts or the notification letters should be directed to the health records clerks.


Any report or dictation issues should be directed to Transcription Services.
