Emailing risks notification

Use of Email to submit your request for health records:

Email communications can be a convenient way to submit your request for health records, but there are potential risks when using email to send personal information.  If you choose to email the completed form to our health organization, you are accepting the risks.    

We will do what we can to confirm that any personal information you send is being sent by you and only you, but it’s never possible to have 100% certainty who we are communicating with through email.   

You need to be aware that we cannot control what happens to your personal information once it is stored: 
  • on your phone, tablet or computer (“Device”); 
  • by telecommunications providers; 
  • by software or application providers; or 
  • by other applications that may have access to your messages.
You are responsible for the security of your own Device and email service.

What are the potential risks?
  • Your information could be requested, viewed, changed or deleted if others are allowed to access your Device or email account.
  • Information may be vulnerable if stored on a Device that has been compromised by viruses or malware.
  • Electronic communications can be intercepted by third parties.
  • Your data may be stored and/or accessed outside of Canada.
