HIM is in a process to centralize services for Researchers to access patient health information; however, current requests for research data may require specific forms to be completed and returned to a specific site.
Research Access Forms
Included below are a number of forms that are available for download for Researchers to request information:

- Fraser Health
- Vancouver Coastal Health
- Provincial Health Services Authority
Please note that fees for retrieving files may vary by health care location.
Additionally, if electronic access to files is required, and you currently are not enrolled in the particular Health Information System for the site, you will need to gain account approval first prior to accessing health information.
